Your files may include your name, contact details, and personal URLs; they will not be automatically rejected for those details. Include only information you want the hiring team to see. Use public or synthetic examples and omit patient data and confidential material.

The form collects your contact email separately from the two work samples. Your form contact email is stored securely and shown only to the authorized hiring reviewer. We email both files as attachments to that reviewer so they can follow up directly if they choose. Reviewer records are limited to authorized Careers Workspace owners. Application records are retained for 12 months after submission.
